Consultations
The practice is open on Tuesday to Friday from 08.00h till 17.00h. On Mondays the practice is opened from 08.00h till 13.00h.
There is no open consultation, so you always need to make an appointment first before visiting the practice. You can do this by calling the assistant between 8.00h and 11.00h. The assistant will ask the reason of your visit, in order to assess the urgency and because of planning reasons. It is only possible to discuss one problem during an appointment, if you experience more then one kind of complaint, we kindly ask you to book a double appointment. The assistant is trained in providing medical advice concerning minor medical problems.
House calls are exclusively intended for patients who cannot travel to the practice for medical reasons. Not having transport to the practice is not a reason for a house call. House calls are made in the early afternoon, so it is important to call the assistant early in the morning when requesting a house call.
When you think you have a urinary tract infection we ask you to deliver a urine sample to the assistant before 10 am. You have to provide a fresh urine sample in a clean container, labeled with your name and date of birth. The assistant will check the urine for a possible infection and discuss the results with the doctor.
You can also plan appointments at the consultation hours of the assistant. She is trained to perform for example:
– taking blood pressure
– ear syringing
– treatment of warts
– performing PAP smears
– removal of sutures
Evenings/weekends/nights
If you require the urgent help of a doctor during the evening (after 17.00h), weekend or night, you may call the Huisartsenpost Amstelland. The Huisartsenpost provides emergency medical care when it is not possible to wait until the next day of after the weekend. Always call the Huisartsenpost before visiting.

Registration
It is possible to register as a new patiënt in our practice by filling in this form (one form per family member), or by picking up a registration package at our desk. Please read the following information carefully.

To complete your registration you have to bring the form in person to the practice (every morning before 12.00h). Also bring copies of identity documents and health insurance cards. The assistent at the desk will check these documents. After completing the registration we will ask your previous GP to transfer your medical file.
It is possible to plan a introductory consult before your registration, in which you will get some information about the practice. In this consult we can also review your medical history, use of medication etcetera. For current medical issues which you want to discuss with the doctor, your registration need to be completed first. It is not possible to plan a regular consult without a completed registration.
Besides the registration form you also need to complete a LSP permission form. This form is meant for the registration of your permission for sharing medical data in urgent situations. The following leaflet will provide you all the information needed about LSP.

Travel information
If you are travelling abroad and need information about vaccinations and malaria profylaxis you have the possibility to get travel advice at our practice. For this service there is a seperate fee, it is not covered by the standard health insurance. For more information click the link below.
Dutch Health Care system
In the Netherlands the huisarts (GP) is the first point of contact if you have a health problem. The GP kan treat routine health issues, perform standard gynaecological and paediatric examinations, and refer you onto other services if necessary, including hospitals, specialists, and physiotherapy.
